Purpose and Objectives
The service provides a comprehensive range of home care, rehabilitation and support services to frail elderly and their carers, so as to facilitate them to maintain their optimal level of functioning and continuous living at home and their familiar community.
Service Boundary
IHCS(F) - Kwai Chung District
Target Groups
1) Elderly persons
- Aged 60 or above;
- Living in the community and not receiving institutional service; and
- Assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services.
2) Carers of Service Users of IHCS(Frail Case) /EHCCS

- Rehabilitation exercises
- Environmental risk assessment and home modification suggestion
- Speech therapy
- Dementia Care
- Meal Service
- Home Respite Service (Day Time)
- Transportation and escort services
- Arrange Respite service
- Counselling service
- Carer support services
- On site carer training
- Carer support programmes
- 24 hour emergency support
The scope and frequency of services will be designed by the IHCS(F)/EHCCS according to the assessed needs of the individual frail elderly persons.
How to apply for and withdraw from the service
Service Application
Elderly persons and their families can approach to their nearest Integrated Family Service Centres, District Elderly Community Centres, Neighborhood Elderly Centres or Medical Social
Services Units for application and arrangement of Standardised Care
Need Assessment to confirm their eligibility for service.
Fees
Fees are charged on a sliding scale with reference to the service users’ financial conditions and may be revised annually.
Withdrawal Procedure
Written or verbal notice is required as soon as possible if service user has decided to withdraw from the service.
