With a focus on community health, the Community Health Promotion Unit (“CHPU”) aims at instilling the holistic health concept into the grassroots community and improving their health by enhancing and building the community health resources network. CHPU provides the grassroots community with health education and information, plans the medical-social collaboration service mode, recruits and trains community health ambassadors. CHPU implemented services sponsored by various funds during the year, including the Jockey Club Community eHealth Care Project the Jockey Club Empowerment-based Transitional Care Project (JCETC), the Jockey Club Cancer Survivorship Care Project (JCCSCP) and the Jockey Club “Confront Pain with Ease” Project (JC-COPE). The department also continued to promote health information and regular services in the form of themed activities and to meet the health needs of the target service users through home visits or online support.
